10/25/2023 EnchantaVaughn Approved (AM) Upon arrival, client was observed greeting peers and adults as he walked inside of his class. Client followed up with momâs contact information with QMHP. Teacher communicated that client is allowed to attend group session everyday after lunch to accommodate his needs in order to meet his behavior expectations. Client expresses that he practices karate at home during his spare time and wants to join a team. Client displays a proactive attitude when talking about weekend activities. Client discussed positive thoughts about being in school today. (PM) Client explained he miss being in pre K because he wasn't required to do a lot of work and they always got to play and take rest breaks throughout the day. Client understands that he needs to adapt to his new environment and continues to demonstrate cooperative behaviors and show interest in his school work. (EOD) Client appears to grasp a clear understanding of expectations and acknowledges his personal strengths and weaknesses through role playing exercises and psychoeducational activities. Client displays appropriate social skills in the community setting and does not have to be reminded of the rules and regulations, achieving his goals and objectives. (AM) QMHP welcomed client to school. QMHP inquired about evening and morning. QMHP asked how client was feeling. QMHP asked if client needed help with anything. QMHP encouraged client to have a good day. (PM) QMHP monitored individualized behaviors for acquisition of objectives. QMHP encouraged client to support his peers. QMHP encouraged client to be the example for others. QMHP encouraged client not to follow others he see that are doing wrong. QMHP reminded client of goals and objectives. (EOD) QMHP reflected with client on his day, reviewing both good and bad experiences. QMHP processed with client, "what would you do different tomorrowâ. QMHP encouraged client to have a good evening. QMHP encouraged client that tomorrow is a new day. QMHP processed client positive responses and challenged client to display appropriate manners at all times. (AM) Client greeted all staff with appropriate gestures (e.g. waving and saying hello). Client said that he had a good rest of the day yesterday and expressed positive thoughts about his morning arrival. Client stated he was excited about group activities. Client said he was good and attended to his assignment. (PM) Client displayed appropriate social behaviors in various settings. Client was on track with goals and objectives. Client was monitored with low impulsivity, seated, and focused on his work. Client understood he is expected to lead by example. (EOD) Client engaged in self reflection. Client was able to role play his behaviors and identify a decrease in ADHD symptoms. Client relates heavily to both negative and positive behaviors when talking about his personal experiences. Client facial expressions demonstrated that he was happy with how the day turned out.
